Thank you for choosing McCaffrey Laboratory for your Integrated Urinalysis Panel testing.

By choosing McCaffrey Laboratory you understand and agree to the following terms and conditions. McCaffrey Laboratory reserves the right to make changes to these terms and conditions as needed. All modifications will be effective immediately upon posting.

• You understand and agree that McCaffrey Laboratory does not provide medical advice and that our services are not intended to diagnose, treat, or cure any disease or condition. Any advice or recommendation given by our doctor’s or staff is for educational purposes only and is not intended to diagnose, treat, or cure any disease or condition. To maintain a non-doctor/patient relationship, you may need to provide medical history during a phone or email conversation with our doctor’s or staff.
• You understand that you do not hold a doctor/patient relationship with any doctor employed by McCaffrey Laboratory. Any information produced (verbally or written) is not intended to diagnose, treat, or cure any disease or condition. You understand that our doctor’s and staff can not give treatment recommendations. You cannot hold McCaffrey Laboratory, or any of its employees responsible for any personal lifestyle changes made after any communication with doctor’s or staff.
• You understand that tests processed by some labs can not be ordered in the states of NY, NJ, MA, or RI. You understand that you can still order these tests, however, you must have the test completed in a non-restricted state.
• You understand and agree that any test recommendations or health information provided from McCaffrey Laboratories, written or verbal (including educational video’s), are for educational purposes only and not intended to diagnose, treat, or cure any disease condition.
• You understand that your test results carry a risk of false negatives and do not definitively define a diagnosis. You understand you cannot be refunded for results that indicate a false positive(s)/negative(s). This also applies to subjective false positive(s)/negative(s). Refunds can be given if there was a sample instability and/or confirmed issue with the labs calibration/machinery.
• You agree that information submitted to McCaffrey Laboratories is truthful and accurate.
• You will keep your password and information questions confidential.
• You will use the McCaffrey Laboratory website for its intended use only.
• You understand you must be 18 or older to have an account with McCaffrey Laboratories, and confirm you are 18 before opening an account with McCaffrey Laboratories.
• You understand that test results can sometimes be inaccurate through no fault of McCaffrey Laboratories.
• You will not copy, and share information provided to you by McCaffrey Laboratories nor use it illegally.
• You will abide by McCaffrey laboratories “order” and “refund” policies. You may cancel your order anytime before the specimen is received. If a refund is requested or required, your refund will be reduced by 20% of the purchase price.
• You agree to indemnify, defend, or hold harmless McCaffrey Laboratories, its officers, directors, staff, agents, licensors, suppliers and any third-party information providers against all losses, expenses, damages, and costs, including reasonable attorneys’ fees, resulting from any violation of this agreement by you.
