HIPAA Notice of Privacy Practices for Personal Health Information

Dear Customer of McCaffrey Laboratory,

We are required to provide you with this Notice of Privacy Practices and to explain our legal duties under the Federal Health Insurance Portability and Accountability Act (HIPAA).

By law we are required to:

  • Maintain the privacy of your Personal Health Information (PHI)
  • Provide you this notice of our legal duties and privacy practices with respect to your PHI; and
  • Follow the terms of this notice.

We ask that all clients of McCaffrey Laboratory use a unique identifier so that patient information is only identifiable buy the Healthcare Provider submitting specimen for processing. An individual’s clinical information is forwarded directly to the Healthcare Provider’s email address that was used for setting up an account. McCaffrey Laboratory retains the information in a secure electronic file which is archived for 3 years.

We protect your information from inappropriate use or disclosure. Our employees are required to comply with our requirements that protect the confidentiality of your information. They may look at your information only when there is an appropriate reason to do so, such as to administer the process of returning your specimen results back to the Healthcare Provider dealing with processing.

We will not knowingly disclose or sell your information to any other individual or organization for their use in marketing products to you without your prior consent.

We will not forward by mail, fax or electronically your information to any other Healthcare Provider other than the ordering client without your written consent.

We reserve the right to change the terms of this Notice of Privacy Practices at any time. Any changes will apply to information we already have and any information we receive in the future. A copy of the new notice will be posted at McCaffrey Laboratory and provided to individuals upon request and required by law. You may request a copy of the current notice at any time.
